3717-1-05.1(D) / Backflow prevention - air gap.
Critical Corrected During Inspection Observed nozzle for prep sink to lay below the rim at the time of inspection. PIC placed it strategically so it would not be below the flood rim until they can get a replacement. Insufficient air gap between the flood rim and the water supply inlet.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Observed bucket of pickles in the walk in cooler without a lid at the time of inspection. PIC voluntarily disposed of pickles due to them being almost empty. Improper storage of food items.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed torn gasket on the walk in freezer at the time of inspection. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of dust on the vent above the pop dispenser in the kitchen at the time of inspection. Observed an accumulation of dust on the vent covers in the walk in cooler. Non-food contact surfaces of equipment are unclean.

Corrected During Inspection Observed mop being stored in mop sink at the time of inspection. PIC hung the mop up to dry properly. Mops dried improperly.
