Poisonous or toxic chemicals did not have manufacturers label. Observed a bottle of bleach in the original container without the manufacturer label.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components not maintained. Observed damaged preparation table. PIC stated a new prep table will be coming to replace the damaged one.

PIC stated they are not cooling or reheating any food anymore. All left over foods are discarded at the end of each day.
