Critical Corrected During Inspection Employee could not locate sick policy. Gave her a copy and had all employees sign while at the facility. Employees are not informed in a verifiable manner of their responsibility to report information about their health.

Critical Corrected During Inspection Observed the ice machine in the self service area to have build up. PIC cleaned and sanitized the ice shoot. Equipment food-contact surfaces or utensils are unclean.

Critical Corrected During Inspection Did not observe a date/time stamp on the tomatoes in the walk in. PIC date marked items. Refrigerated, ready-to-eat, TCS foods not properly date marked.

Did not observe a thermometer in the chicken fridge. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.
