3717-1-03.6(A) / Discarding/reconditioning unsafe, adulterated, or not honestly presented food
Critical Corrected During Inspection Observed two cans of can of Bush's beans with dents. The person-in-charge voluntarly removed them from the shelf.
To prevent the transmission of foodborne illness, a food that is unsafe, adulterated, or not honestly presented as specified under 3717-1-03 of this code shall be discarded or reconditioned according to an approved procedure.
3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Observed a hand washing sink in the deli and produce department with visible food debris. Hand sinks should remain clean at all times to properly wash hands.
To prevent contamination, a handwashing sink may not be used for purposes other than handwashing.
3717-1-08.3(H) / Special requirements: bulk water machine criteria - Sampling
Critical Repeat Corrected During Inspection Water dispensed from bulk water machine has not been sampled at the required frequency, and/or sample results were not retained at the facility. Please fax these results to the health department at 740-383-2251.
To prevent illness, the water dispensed from bulk water machines shall be sampled at least semi-annually for coliform bacteria. Sample results shall be retained at the FSO or RFE for review by the licensor.

3717-1-03.5(B) / Honestly presented.
Observed ham labels in the deil without the full proper name. (ex. chopped ham water added/ ham off the bone with natural juices etc.).
Food shall be offered for human consumption that does not mislead or misinform the consumer. Food or color additives, colored overwraps, or lights may not be used to misrepresent the food. The sale of kosher foods shall be in accordance with section 1329.29 of the Revised Code.
3717-1-04.2(I) / Sanitizing solutions - testing devices.
Repeat There is no test kit available for measuring the concentration of the sanitizer in the bakery and produce 3 bay sink.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed excessive ice build-up in the ice-cream freezer. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the hot water fryer not functioning properly.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed several vents with visible build-up. The surrounding ceiling tiles are soiled.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.
