Repeat Observed torn gasket on the make table near the grill during the time of inspection. Equipment components are not intact, tight or properly adjusted.

Repeat Observed bottom self of table to be entirely covered in rust during the time of inspection. Non-food contact surfaces of equipment are unclean.

Repeat Observed several damaged ceiling tiles and floor tiles throughout the facility during the time of inspection. Physical facilities not maintained in good repair.
