Critical Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. The facility has an employee health policy but it was not verifiable. PIC had employees go through the sick policy and sign it.

Facility does not have a person with level two certification in food protection. PIC has level 2 servsafe but do not have the ODH certificate.

Food thermometer not readily accessible. Did not observe a thermometer inside the walk in coolers at the deli, dairy, produce and meat. PIC made a note to get a thermometer inside all walk in coolers.

Equipment and/or components not maintained. Observed ice build up inside the walk in freezer at the deli and excessive ice build up inside the big walk in freezer in the back room.

Non-food contact surfaces of equipment are unclean. Observed the vents cover at the deli walk in cooler and walk in freezer with build up.
