Critical Corrected During Inspection Did not observe signed sick policies during the time of inspection. Employees are not informed in a verifiable manner of their responsibility to report information about their health. Being that there are very few employees in the facility, I supplied the 3 employees in the facility with sick policies and had them sign it. PIC will need to ensure that other two employees sign the black copies I supplied them with.

Critical Repeat Corrected During Inspection Observed raw chicken stored above beef and pork during the time of inspection. Food not properly protected from contamination by separation, packaging, and segregation. PIC rearranged items so they were properly stored during the time of inspection.

Critical Corrected During Inspection Observed lid on container with chicken in it to be cracked during the time of inspection. Food not properly protected from contamination by separation, packaging, and segregation. PIC placed new lid on container during the time of inspection.

Critical Corrected During Inspection Observed chemical bottle without common name on it during the time of inspection. Working containers of poisonous or toxic materials not properly labeled. PIC placed common name on bottle during the time of inspection.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Did not observe written procedure for responding to vomiting and diarrheal events during the time of inspection. No written procedures for responding to vomiting or diarrheal events.

3717-1-04.1(Y) / Temperature measuring devices.
Did not observe internal thermometer in stand-up cooler during the time of inspection. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

PIC stated they plan on selling the facility during the time of inspection. PIC will need to contact the Marion Public Health Department if they follow through with doing so.
Last water test: 9/19. Results have not came back yet. Results will be faxed to health department.
