3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Corrected During Inspection Observed a container stored in the hand sink at the bar and empty boxes stored by the hand sink making it not accessible. PIC removed the items. Handwashing sink not accessible.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Corrected During Inspection Observed open bag of shredded cheese in the cooler of the upstair kitchen, open bag of cabbage and cut onions in the cooler of the kitchen downstair without date marking. PIC discarded them. Refrigerated, ready-to-eat, TCS foods not properly date marked.

3717-1-02.4(A)(2) / PIC - Manager Certification
Facility has one employee with manager certificate but did not have the ODH certificate. A copy of the reciprocity form was given to PIC. Facility does not have an employee with manager certification in food protection.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
Observed expired test kit at the bar and kitchen upstair. Observed water damaged test kit in the kitchen downstair. No sanitizer test kit available.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed build up on the shelves on the make table in the upstair kitchen. Non-food contact surfaces of equipment are unclean.
