[?] A summary of the violations found during the inspection are listed below.

3717-1-04.1(Y) / Temperature measuring devices.
Observed non-working thermometer in one section of the produce reach in case and in the deli hot holding case. Deli cold and hot holding cases need thermometers inside. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed dripping condensation in the meat walk in cooler. PIC stated it was dripping before and a work order was submitted. Maintenance came and fixed it and it has started dripping again. Another work was submitted during inspection and a copy of the work order was provided. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed dust build up on the fan covers of the deli walk in cooler and walk in freezer. Observed debris on the floor of the deli walk in freezer and milk spill on the floor of the dairy walk in cooler. PIC had employees clean the floor of the deli walk in freezer and dairy walk in cooler. Non-food contact surfaces of equipment are unclean.

901:3-4-02(G) / RFE must display license
Observed current license not displayed at the time of inspection. A copy of the license will be sent to the facility. RFE license not posted in the facility.
