Repeat Facility did not have an ODH manager certificate on file during the time of inspection. Facility does not have an employee with manager certification in food protection.

3717-1-04.1(Y) / Temperature measuring devices.
Repeat Observed two reach-in coolers to not have internal thermometers during the time of inspection. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Repeat Did not observe food containers in storage to be labeled with common names of food during the time of inspection. Working food containers not properly labeled.

3717-1-03.2(M) / Wiping cloths - use limitation.
Repeat Observed wiping cloths throughout the kitchen to be stored improperly during the time of inspection. Improper use and/or maintenance of wiping cloths.

3717-1-04.8(E)(2) / Clean equipment and utensils stored in a self-draining position and covered or inverted.
Repeat Observed items on drying racks to not be inverted during the time of inspection. Clean equipment and utensils not stored in a self-draining position and covered or inverted.

Repeat Observed single-use straws at bar to not be individually wrapped during the time of inspection. Single-service and single-use articles not protected from contamination.

Repeat Observed cooler drawer below the grill to not be sealing shut entirely during the time of inspection. Equipment components are not intact, tight or properly adjusted.

Repeat Observed rim of produce sink to have debris build-up during the time of inspection. Non-food contact surfaces of equipment are unclean.

Observed the dishwashing machine to not be distributing chlorine during the time of inspection. Chlorine sanitizing solution at incorrect temperature and/or concentration. Facility will be using three bay sink until further notice.
