[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical TCS foods were not being held at the proper temperature. Hot dogs in reach in at 49 F.
Person in charge disposed of hot dogs. Except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except for those roasts that are cooked as required may be held at 130° F), or at 41°F or less.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items. Boxes on floor of walk in refrigerator.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-03.4(C) / Thawing - temperature and time control.
TCS foods were not properly thawed. Chicken being thawed in hot water.
TCS food shall be thawed as required in this rule. Person in charge placed on ice and in refrigeration.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. Ice machine in back has corrosion above ice bin, debris on ledge can fall into ice bin.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. Ice cream machine cover bent, leaving a gap at top.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. Reach in refrigerator by grill was at 60 degrees F. Hot dogs in unit were reading internal temperature of 49 degrees F.
Person in charge removed items from unit and put sign on door to not use until working properly. Hot dogs were disposed of. Will be placing a work order. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed ice cream mix on racks above ice cream machine from splashing.
