3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed ice machine with build up. PIC cleaned it.

3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Corrected During Inspection Working containers of poisonous or toxic materials not properly labeled. Observed a spray bottle of degreaser and a bottle of bleach water without label. PIC labeled it.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Working food containers not properly labeled. Observed bottles of sluchi mix in the kitchen without label.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed the freezer in the dry storage area not working. PIC stated they just found out the freezer has stop working and all food gone bad. PIC was emptying and discarding all food items in the freezer at the time of inspection. Talked to PIC about having maintenance come and check the freezer and make sure it is in good repair and working before reusing it.

Non-food contact surfaces of equipment are unclean. Observed build up on the shelves and bottom of the cooler in the dry storage area. Also observed devris in the microwave and nacho cheese dispenser in the kitchen.
