3717-1-03.5(C)(2) / Food labels - packaged food
Observed no ingredient stated on the banana bread. A label should contain the common name of the food, list of ingredients, net weight, name and place of business of the manufacture and major allergens.
Foods shall be labeled as specified in this rule.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed a torn gasket on the Aladdin cooler gasket.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Corrected During Inspection Observed a handwashing sink, next to the office, without water at the required temperature.
To ensure proper cleaning of hands, a handwashing sink shall be equipped to provide water at a temperature of at least 100¡ F (thirty-eight degrees Celsius) through a mixing valve or combination faucet.

Observed no handwashing sign(s) posted at handwashing sink(s) used by employees.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
