Repeat Toilet room not completely enclosed and/or did not have tight-fitting, self-closing door. Observed the toilet room door that is not self-closing.

Repeat Physical facilities not maintained in good repair. Observed damaged brick wall behind the oven and damaged floor tiles underneath the oven.
