3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed boxes of packaged bacon stored above blue cheese and feta cheese. Person-in-charge moved the bacon to the bottom. Observed food that was not properly protected from contamination by separation, packaging, and segregation.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.

3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Repeat Observed several plastic lids and pans with cracks and/ or corners broken off. These dishes can no longer be sanitized properly. Equipment and/or utensils contain improper materials.
To prevent contamination, materials that are used in the contruction of utensils and food contact surfaces may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: Safe; durable, corrosion-resistant, and nonabsorbent; Sufficient in weight and thickness to withstand repeated warewashing; finished to have a smooth, easily cleanable surface; and resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.
3717-1-04(I) / Nonfood-contact surfaces - materials.
Repeat Observed shelves in the reach in cooler with rust. The shelves are no longer smooth and easily cleanable. Observed nonfood-contact surfaces constructed of unapproved materials.
Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be smooth and durable.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed the fan unit in the Master-Bilt cooler with visible build-up. Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Corrected During Inspection Observed several single use lids not properly inverted to help prevent contamination. An employee flipped the lids over. Cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored.
Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule.
3717-1-06.4(A) / Repairing.
Observed some ceiling tiles in the dinning room with water damage. The physical facilities are not being maintained in good repair.
The physical facilities shall be maintained in good repair.
