Critical Corrected During Inspection Observed bottle of sanitizer with no label on it during the time of inspection. Working containers of poisonous or toxic materials not properly labeled. Employee wrote common name on bottle.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed back door gasket to be torn off during the time of inspection. Equipment components are not intact, tight or properly adjusted.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Observed cutting board that is used to set cups on, not food, to not be smooth and easily cleanable. Nonfood-contact surfaces not easily cleanable.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
Corrected During Inspection Did not observe test strips for the sanitizer during the time of inspection. No sanitizer test kit available. Gave PIC test strips before leaving.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed back cooler to have a large build-up of debris and standing water during the time of inspection. Non-food contact surfaces of equipment are unclean.

Observed damaged and cracked areas in the floor during the time of inspection. Floors, walls, and/or ceilings not smooth and easily cleanable.
