3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Observed food displays that did not have food stored 6 inches above the floor (peanut butter, black beans, tomato sauce, chips, pretsels, cheese balls, raisins etc.).
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Repeat Observed multiple parts used with the meat grinder rusty.
To prevent contamination, materials that are used in the construction of utensils and food contact surfaces may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: Safe; durable, corrosion-resistant, and nonabsorbent; Sufficient in weight and thickness to withstand repeated warewashing; finished to have a smooth, easily cleanable surface; and resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.
3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed a non-commercial microwave in the deli/ bakery. Equipment is not approved by a recognized testing agency.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed damaged coving in the dairy walk-in cooler.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed build-up on several of the walk-in cooler fan units (deli, meat, dairy etc.). Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-06.1(M) / Outer openings - protected.
Repeat Observed a gap in the door sweep by the back recieving door along with the front entry doors.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.

Observed build-up inside the 3-door bakery freezer. Non-food contact surfaces of equipment are dirty.

3717-1-06.4(K)/Controlling pests.
Observed a live bird in the facility during the inspecton. The bird was sitting on exposed food in the produce area. Copesan pest solution was in on 12/28/18 but was not able to capture the bird. Please contact pest operator to get rid of the bird.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions.
901:3-4-15(B)/Embargoing of food due to adulteration, misbranding, or expired baby formula or baby foods
Observed banana blueberry baby food that was expired 1/18/2018 that was still on the shelf. The person-in-charge voluntary discarded the baby food.
To prevent foodborne illness, whenever a licensor finds or has cause to believe within an RFE or FSO in their jurisdiction that any food is adulterated, or so misbranded as to be dangerous or fraudulent, or that infant formula, or baby food is expired, the licensor shall affix to the item a tag giving notice that the item is, or is suspected of being, adulterated, misbranded, or expired and has been embargoed.

Observed a handwashing sink in the mens restroom needed recauked.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule.
3717-1-04.4(A)(1)/Equipment - good repair and proper adjustment.
Observed the raw chicken walk-in cooler door jam needing repaired. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-05.1(J)/Backflow prevention device - when required.
Observed the deli dish sprayer that was hanging below the rim of the sink.
To prevent contamination, a plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO or RFE, including on a hose bibb if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by the Ohio building code, by: (1) Providing an air gap as specified under paragraph (D) of this rule; or (2) Installing an approved backflow prevention device as specified under paragraph (E) of this rule.
3717-1-05.1(S)(1)/Plumbing system - maintained in good repair.
Observed a toliet in the mens restroom that was out of order.
3717-1-06.4(A)/Repairing.
Observed water damaged ceiling tiles by the popcorn and beer displays. The physical facilities are not being maintained in good repair.
