Critical Corrected During Inspection Observed a few live flies in the kitchen beverage area and a couple live cockroach nymphs in the cleaning closet. Facility already has a pest control contract in place with a pest control company. PIC was advised during the inspection to reach out to the company to perform additional extermination actions in these areas. Presence of live insects, rodents, and other pests.

Observed build-up of frost in the walk-in freezer, chicken nugget freezer, and fry freezer, as well as torn gaskets on the breakfast cooler and dessert cooler. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed build-up of debris on the breakfast cooler gaskets, in the dessert cooler, on the backsplash of the grills, and on vent covers in the restrooms. Non-food contact surfaces of equipment are unclean.

Observed mop sink faucet leaking. Plumbing system not properly maintained or repaired.

Observed damaged floor and wall tiles as well as small holes in walls throughout the facility. Floors, walls, and/or ceilings not smooth and easily cleanable.
