Critical Corrected During Inspection Observed milk gallon to be damaged in some way during the time of inspection. The gallon container had milk on it, and it was hard to tell if milk was spilled on the package or if the packaged had leaked during the time of inspection. Food package(s) received in poor condition. PIC removed milk container from shelves during the time of inspection.

Observed paper towel dispenser at hand sink in produce area to not be functioning properly during the time of inspection. No towels or drying device at the handwashing sink(s).

3717-1-04.4(Q)(2) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Observed thermometer on mechanical dishwashing machine in the bakery to not be functioning properly during the time of inspection. Utensils, thermometers, and/or pressure gauges not in good repair or calibrated.

3717-1-06.1(M) / Outer openings - protected.
Observed door sweep on exit door near the dairy cooler to be damaged during the time of inspection. Outer opening not protected.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed dairy cooler gasket, meat cooler gasket, and freezer A35 gaskets to be torn during the time of inspection. Equipment components are not intact, tight or properly adjusted.

Observed cracked floors throughout the produce area during the time of inspection. Floors, walls, and/or ceilings not smooth and easily cleanable.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed warewashing area of bakery to have excessive build-up on walls during the time of inspection. Facility not maintained clean.

Repeat Observed damaged ceiling tiles in the deli area during the time of inspection. Physical facilities not maintained in good repair.

Observed there to be no lights functioning in several freezers on the consumer floor during the time of inspection. Light intensity less than twenty foot candles in required areas.
