Critical Corrected During Inspection Observed multiple cleaned dishes that still had debris on them. PIC took the dishes back to the dishwashing area to be rewashed and will retrain staff on dishwashing. Equipment food-contact surfaces or utensils are unclean.

Critical Observed 2 live cockroaches as well as a dead cockroach in the dishwashing area of the kitchen. PIC has a weekly service contract with their pest control company and is having the area sprayed on 11/1/23. Presence of live insects.

Critical Observed multiple damaged and partially melted plastic lids being used to store food. Food contact surfaces not easily cleanable.

Observed washed dishes being stacked wet. Equipment and utensils are not being air dried.

Repeat Corrected During Inspection Observed some single-service fryer baskets and clean dishes stored face-side up.

Observed a torn gasket on the expo cooler. Equipment components are not intact, tight or properly adjusted.

Observed the kitchen hand sink with cracked caulk. Fixed equipment not properly sealed or spaced for cleaning.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed a build-up of debris on the walk-in cooler gasket, on the fryer cooler and freezer gaskets, around the hand sink in the dishwashing area and women's restroom, on the food warmers, inside the fry cooler and freezer, and on the shelving unit near the fryers. Non-food contact surfaces of equipment are unclean.

Observed a build-up of debris on the kitchen walls and on the floor in the men's restroom.

Observed holes and cracks in the kitchen floor tiles as well as in the baseboards throughout the kitchen. The stainless steel wall beside the 3-bay sink was observed to be torn up along the bottom, and a hole was observed in the wall in the liquor closet. Floors, walls, and/or ceilings not smooth and easily cleanable.
