3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed raw bacon being stored above milk in the self service case.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Corrected During Inspection Handwashing sink is being used for purposes other than handwashing. Observed damaged goods placed in the handwashing sink
To prevent contamination, a handwashing sink may not be used for purposes other than handwashing.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed cups in the back storage area stored directly on the floor.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed vents in the store and the self service case soiled, the fan guards in the walk-in cooler soiled and the Arctic air freezer with ice build-up and visible food debris .
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-04.8(G) / Kitchenware and tableware - handling.
Corrected During Inspection Observed single service coffee filters on top of the creamer dispenser.
Single-service articles or single-use articles and cleaned and sanitized utensils shall be handled, displayed, and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulb in the walk-in cooler not properly shielded or coated where required.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
3717-1-06.3 (C)(1) / Employee accommodations - designated areas
Corrected During Inspection Observed a coffee cup being stored on a food contact shelf in the walk-in cooler.
Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination.
3717-1-06.4(F) / Drying mops.
Observed a wet mop being stored in a mop bucket.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
