3717-1-02.4(A)(2) / PIC - Manager Certification
Did not observe an ODH manager certificate on file in the facility. Facility does not have an employee with manager certification in food protection. Gave PIC paper they need to receive their certificate from ODH.

3717-1-04.4(Q)(2) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Corrected During Inspection Observed two damaged thermometers in the milk cooler and the cooler in the cafeteria during the time of inspection. Utensils, thermometers, and/or pressure gauges not in good repair or calibrated. PIC made sure there were functioning thermometers in the coolers.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed torn gasket on the upright cooler in the kitchen during the time of inspection. Equipment components are not intact, tight or properly adjusted.

3717-1-06.1(A) / Floors, walls, and ceilings.
Observed damaged floor tiles under the prep table by the back door during the time of inspection. Floors, walls, and/or ceilings not smooth and easily cleanable.
