3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection Observed beef being held at 46 degrees at the time of inspection. PIC corrected the violation by throwing out the meat. TCS foods not being cold held at the proper temperature.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Observed mop sink and drain by coffee station with an insuffcient air gap at the time of inspection. PIC has notified maintenance. Will reinspect within a week. Insufficient air gap between the flood rim and the water supply inlet.

3717-1-04.1(Y) / Temperature measuring devices.
Observed upright cooler with no internal thermometer at the time of inspection. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed walk in freezer and wall freezer with a build up of ice at the time of inspection. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed multiple coolers with gaskets with a build up of debris. Non-food contact surfaces of equipment are unclean.

Observed dumpster without drain plug. No drain plugs in waste receptacle.

Observed vents in the stalls of both bathrooms with an accumulation of debris at the time of inspection. Attachments to the wall and/or ceiling were not easily cleanable.

Observed walk in cooler, back storage room and kitchen floor with debris at the time of inspection. Facility not maintained clean.
