Critical Did not observe a signed sick policy on file during the time of inspection. Employees are not informed in a verifiable manner of their responsibility to report information about their health. Provided PIC with sick policy and will be following up to ensure it is signed.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Corrected During Inspection Observed several items being stored in the hand sink during the time of inspection. Handwashing sink is being used for purposes other than handwashing. PIC removed items.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical Corrected During Inspection Observed hot holding unit to be sitting at 120 degrees during the time of inspection. TCS foods not being hot held at the proper temperature. PIC discarded foods in the unit and turned the temperature of the unit up.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Observed several ready-to-eat items in the front cooler to be held past their disposition date during the time of inspection. Ready-to-eat, TCS food not properly discarded when required. PIC disposed of foods.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Corrected During Inspection Observed several foods to be missing date marks during the time of inspection that had been held for more than 24 hours. Refrigerated, ready-to-eat, TCS foods not properly date marked. PIC disposed of them.

Critical Observed cake pops in bulk food container to not have allergen declarations in the allergen packet during the time of inspection. Missing and/or incomplete allergen declaration(s).

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Corrected During Inspection Did not observe an accessible, written procedure for responding to vomiting or diarrheal events in the facility during the time of inspection. No written procedures for responding to vomiting or diarrheal events. Provided PIC with a written policy before leaving the facility.

3717-1-04.1(Y) / Temperature measuring devices.
Did not observe a thermometer in the cooler located in the food prep area during the time of inspection. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-04.1(KK) / Food equipment - certification and classification.
Observed a noncommercial cooler being used in the front of the facility during the time of inspection. Equipment not approved by a recognized testing agency.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed hand sink to need coved to the wall as the coving was detached from the wall during the time of inspection. Floor and wall junctures not properly coved or closed and/or floor drains not provided.
