Critical Corrected During Inspection Observed bottles stored in the handsink. PIC moved items out of handsink. Handwashing sink is being used for purposes other than handwashing.

Critical Corrected During Inspection Observed apple crisp being stored on the floor of the walk in cooler. PIC moved food to shelf. Food not properly protected from contamination by separation, packaging, and segregation.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Observed bar 3 bay sink and kitchen 3 bay sink to have too high of sanitizer concentration. PIC drained each sink and refilled with correct sanitizer. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness.

Critical Corrected During Inspection Observed spicy mustard and lettuce without a proper date mark. Lettuce was dated and mustard was discarded. Refrigerated, ready-to-eat, TCS foods not properly date marked.

3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Corrected During Inspection Observed cutting board with deep grooves. PIC flipped cutting board to new side. Food contact surfaces not easily cleanable.

3717-1-07.1(P) / Personal care items - storage.
Observed employee drinks and personal items not stored in a designated area. Personal care items stored improperly.

3717-1-04.1(Y) / Temperature measuring devices.
Repeat Corrected During Inspection Observed make table, hot holding cabinet, and prep cooler without internal thermometers. PIC placed thermometers inside units. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed mini fridge, walk in freezer and cooler under prep table with torn gaskets. Equipment components are not intact, tight or properly adjusted.

Repeat Observed dirty gaskets throughout the facility. Non-food contact surfaces of equipment are unclean.

Corrected During Inspection Observed mop not hanging to dry. PIC hung mop. Mops dried improperly.

Observed broken floor tiles in the kitchen. Floors, walls, and/or ceilings not smooth and easily cleanable.
