3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Observed salsa pollo sauce made on 10/10 still in the walk in cooler. PIC discarded it. Ready-to-eat, TCS food not properly discarded when required.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed uncovered food items in the make table by the grill. Improper storage of food items.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed broken doors on the walk in cooler and freezer. Screwdrivers are still used to latch the doors. Observed the walk in freezer leaking water onto the floor outside of the unit. PIC stated maintenace is supposed to come and fix it. Also observed rusty floor in the wal in freezer. Floor needs repaired. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed debris in the gaskets and inside the make tables in the kitchen. Observed the dry storage rack needing cleaning. Non-food contact surfaces of equipment are unclean.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed standing water on the floor by the 3 compartment sink. Water is dripping on the floor from the mop being improperly hanged to dry. Mop shall be hanged in the mop sink to drain. Also observed water dripping on the floor from the wallk in freezer. Facility not maintained clean.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Onserved slight dust build up on the fan cover above the make table in the kitchen where single use articles are stored. Ventilation system not maintained.
