3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Inside back of the freezer, bottom of the oven inside and outside, refrigerator inside bottom and doors and door seals had mold and/or old product build-up.
To prevent possible pathogen growth, equipment food-contact surfaces and utensils shall be clean to sight and touch. Correct By: 01-Aug-2014

Observed spillaage of cheese and other food particles between equipment and on the floor.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
