3717-1-06.4(A) / Repairing.
Observed an area in the vestibule that is in need of repair. The physical facilities are not being maintained in good repair.
The physical facilities shall be maintained in good repair.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Repeat Observed vents with build-up. The ventilation system was not being properly cleaned or was creating a public health hazard.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.

Comments
Complainant stated the restaurant is not clean. The overhead vents are black with dirt, mold growing on floor by windows on N East corner, the rest-rooms are consistently dirty, walls and caulking around fixtures deteriorated, employee indicates lack of consistency in expired dates on food, floors are not swept, tables not cleaned and when they are the same towel is used with the chairs and flies found in bagel area. Complainant stated a true lack of health concern.

Discussed the complaint with the person-in-charge. Conducted an inspection of the facility. Visible build-up was observed on the vents in the dinning room and in the kitchen. Observed a damaged wall, baseboards and floor tile in the vestibule area. At the time of inspection both restrooms were clean. All foods had a correct date mark. The PIC stated that all products are discarded within two days. At the time of inspection all products were properly date marked. The floors in the kitchen did not appear to have excessive build-up from not cleaning daily.
