[?] A summary of the violations found during the inspection are listed below.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed trying mats to be used for clean dishes during the time of inspection. Equipment food-contact surfaces or utensils are unclean. These mats were dirty to sight and touch during the time of inspection, and cloth fabrics such as these harbor a lot of bacteria. Spoke with employee about coming up with a new space for clean dishes to be stored.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Employees stated to me that they regularly cook food in bulk, freeze it, then that and re-heat it. This was being done for two items that were thawing in the cooler during the time of inspection. This facility is NOT allowed to engage in the act of cooling down and reheating foods as they are only a level three license and they would need to obtain a level four license to do so. Ready-to-eat, TCS food not properly discarded when required. PIC disposed of foods during the time of inspection.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Corrected During Inspection Observed several foods in coolers to not have date marks during the time of inspection. Refrigerated, ready-to-eat, TCS foods not properly date marked. Employee placed date marks on items.

3717-1-04.1(Y) / Temperature measuring devices.
Did not observe internal thermometer in the make table during the time of inspection. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-06.1(K) / Insect control devices - design and installation.
Several of the fly trap mechanisms were too close to food prep areas during the time of inspection. Improper use or placement of insect control devices. These devises should be kept handing away from food prep areas.

3717-1-03.2(M) / Wiping cloths - use limitation.
Observed wiping cloths to be stored improperly during the time of inspection. Improper use and/or maintenance of wiping cloths.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Observed plastic plates to be used as cutting surfaces during the time of inspection. Non-durable equipment observed.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed make table cooler to have excessive build up on the bottom portion during the time of inspection. Non-food contact surfaces of equipment are unclean.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed several pieces of equipment in the front area to not be in use during the time of inspection. Unnecessary or nonfunctional items and /or litter on premises. These items need to be fixed or disposed of due to the fact that they are preventing the front area of the facility from being cleaned entirely.

Observed floors and walls to be damaged during the time of inspection. Physical facilities not maintained in good repair.
