3717-1-03.4(I)(2) / Time as a public health control - four hour time limit
Critical Corrected During Inspection Did not observe an expiration time on the cut french fries in the kitchen. PIC Stated ther were out for about hours and put a time of expiration on the container. Improper use of time as a public health control (4 hours).

3717-1-03.2(M) / Wiping cloths - use limitation.
Repeat Corrected During Inspection Observed a sani bucket in the kitchen needing replaced. PIC replaced it. Improper use and/or maintenance of wiping cloths.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed torn gasket on the door of the dressings make table. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed build up and debris in the gaskets, on the shelves and bottom of the freezer and coolers in the kitchen. Non-food contact surfaces of equipment are unclean.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed dust accumulation on the vent cover in the dry storage. Ventilation system not maintained.
