3717-1-04.1(Y) / Temperature measuring devices.
Did not observe a thermometer in front hot holding device during the time of inspection. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-03.5(C)(2) / Food labels - packaged food
Observed salads packaged in-house to not have required label information during the time of inspection. Label on a food packaged in-house did not contain all required information. These labels should include: common name, ingredients, and net weight/count.

3717-1-06.1(A) / Floors, walls, and ceilings.
Observed floor tiles near the entry/exit door to be damaged during the time of inspection. Floors, walls, and/or ceilings not smooth and easily cleanable.

Note: PIC stated that facility would be purchasing new mechanical ware washing equipment. PIC should be sure to notify the health department once this happens and send the make/model number and/or the proper specification sheet for said machine.

X - P - Chemical: Toxic materials are properly identified and stored.
