Repeat Corrected During Inspection Observed plates that were not covered or inverted at the front. PIC covered them. Clean equipment and utensils not stored in a self-draining position and covered or inverted.

Repeat Observed condensation and ice build up in the walk in freezer. Equipment components are not intact, tight or properly adjusted.
