3717-1-02.4(C)(16) / PIC: duties - ensure written procedures and plans are maintained and implemented in the facility.
Critical The Norovirus cleanup policy and the verifiable employee sick policy were not presented when asked during the inspection. Written procedures and plans are not maintained and implemented.

3717-1-03.2(J) / Food contact with equipment and utensils
Critical Observed the rotisserie chicken metal racks had a build up of black debris on them. Food in contact with an unclean surface or linens.
The soiled racks shall not be used to cook food on until properly cleaned and sanitized.

3717-1-02.4(A)(2) / PIC - Manager Certification
The PIC did not present food safety certification during the inspection. Facility does not have an employee with manager certification in food protection.

3717-1-04.1(Y) / Temperature measuring devices.
Repeat Observed a majority of coolers throughout the store that do not have a thermometer in them. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

Observed the metal racks in the walk in bakery cooler were rusted and had paint chipping off them. Non-food contact surfaces of equipment are unclean.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed the vents in the food service walk in freezer had a large buildup of black debris. Ventilation system not maintained.
