3717-1-02.4(B)(2)(g) / PIC: Demonstration of Knowledge - Stating required temperatures and times for safe storage, holding, cooling, reheating of TCS food
Critical Corrected During Inspection When asked PIC about cooling and reheating procedure and temperature, PIC was unable to demonstrate knowledge for safely cooling and reheating. Educated PIC on cooling and reheating procedures at the time of inspection. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating.

3717-1-02.4(C)(16) / PIC: duties - ensure written procedures and plans are maintained and implemented in the facility.
Critical Corrected During Inspection Observed chili, shredded lettuce and sliced tomatoes with use by date 12/10 in the walk in cooler and cooler in the kitchen at the time of inspection. PIC stated they were made on 12/10 and are usually kept daily. Facility was not folllowing their date marking procedure. Written procedures and plans are not maintained and implemented.

Repeat Facility does not have an employee with manager certification in food protection.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed torn gasket on the door of the 2 door freezer. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed debris inside the gaskets of the freezers. Also observed build up on the fan cover of the walk in cooler.Non-food contact surfaces of equipment are unclean.

3717-1-06.1(I) / Light bulbs - protective shielding.
Observed missing light shield above the 3 bay. No protective shielding on lights.
