| 3717-1-04.2(G)(1)/Food temperature measuring devices - availability Did not observe a thermometer in the 4 door cooler and in the hot box in the kitchen. Food thermometer not readily accessible. |
| 3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment. Observed build up and debris inside the gaskets, on the shelves, on the doors and on the inside of coolers, freezers and hot box throughout the facility. All equipment need cleaned. Non-food contact surfaces of equipment are unclean. |
| 3717-1-06.4(K)/Controlling pests. Observed the presence of live rodents during the inspection. Observed 2 mice inside the mop closet during inspection. Talked to PIC about getting a pest control company in the facility. PIC is given 24 hours to have a pest control company come and assess the issue and take steps into resolving it. A reinspection will be conducted and proof that a pest control company was at the facility shall be provided. Presence of live insects, rodents, and other pests. |

| We went to the facility on 12/14 for reinspection but PIC was not at the facility and we could not get a hold of him. Went back on 12/15, observed ice shoot has been cleaned, thermometer was placed in the 4 door cooler and hot box and coolers/freezer were cleaned during inspection. PIC stated light bulb was ordered for the dry storage. PIC stated pest control was not at the facility on 12/14 and rescheduled for 12/17. A follow up inspection will be done in 2 weeks, on 12/29 to check the pest control issue. |
