Critical Observed raw eggs stored above packaged hashbrowns in the dairy department. Observed food that was not properly protected from contamination by separation, packaging, and segregation.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Observed the sanitizer in the deli above the required ppms.

Critical Observed build-up on the produce sprayers. Equipment food-contact surfaces or utensils are dirty.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
3717-1-04.5(B) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical When asked a deli employee how often the deli slicers are completly torn apart the employee stated at the end of the day. The slicers need to be washed, rinised, and sanitized once every four hours. Food equipment surfaces are not cleaned at the required frequency.
Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified in this rule.
3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Corrected During Inspection Observed a sink in the deli that was was clogged due to something being dumped in this sink. Person-in-charge cleaned out the sink. Handwashing sink is being used for purposes other than handwashing.
To prevent contamination, a handwashing sink may not be used for purposes other than handwashing.
3717-1-08.3(H) / Special requirements: bulk water machine criteria - Sampling
Critical Repeat Water dispensed from bulk water machine has not been sampled at the required frequency, and/or sample results were not retained at the facility. Please fax the most recent sample to the health department at 740-383-2251.
To prevent illness, the water dispensed from bulk water machines shall be sampled at least semi-annually for coliform bacteria. Sample results shall be retained at the FSO or RFE for review by the licensor.

3717-1-03.2(M) / Wiping cloths - use limitation.
Observed wiping cloths that were not completely submerged in a sanitizer solution in Starbucks. Observed improper use and/or maintenance of wiping cloths.
(1) Cloths that are in use for wiping food spills from tableware and carry-out container that occur as food is being served shall be maintained dry and used for no other purpose; (2) Cloths that are in use for wiping counters and other equipment surfaces shall be: (a) held between uses in a chemical sanitizer of the appropriate concentration; and (b) shall be laundered daily; (3) Cloths that are used with raw animal foods shall be kept separate from cloths used for other purposes; (4) Dry wiping cloths and the chemical sanitizing solution in which wet wiping cloths are held between uses shall be free of food debris and visible soil; (5) Containers of chemical sanitizing solutions specified in paragraph (M)(2)(a) of this rule in which wet wiping cloths are held between uses shall be stored off the floor and used in a manner that prevents contamination of food, equipment, utensils, linens, single-service articles, or single-use articles; (6) Single-use disposable sanitizer wipes shall be used in accordance with EPA-approved manufacturer's label use instructions.
3717-1-03.5(B) / Honestly presented.
Repeat Observed sale tags for the hams in the deli without the full name of the product. Example Black Forest Ham with natural juices, Cooked Ham with water product etc. Food products are not honestly presented.
Food shall be offered for human consumption that does not mislead or misinform the consumer. Food or color additives, colored overwraps, or lights may not be used to misrepresent the food. The sale of kosher foods shall be in accordance with section 1329.29 of the Revised Code.
3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Observed plastic dishes in the seafood department that were cracked. These dishes can no longer be washed, rinised, and sanitized. Equipment and/or utensils contain improper materials.
To prevent contamination, materials that are used in the contruction of utensils and food contact surfaces may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: Safe; durable, corrosion-resistant, and nonabsorbent; Sufficient in weight and thickness to withstand repeated warewashing; finished to have a smooth, easily cleanable surface; and resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed torn gaskets on the dairy department storage case doors. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of dust on the walls and ceilings in the deli walk-in cooler. Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean.

Repeat Observed dust on the air vents.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.
