Critical Observed ice machine exposed to dust or contamination from the ceiling tiles that were removed. Ice machine will need to be emptied and cleaned. Equipment food-contact surfaces or utensils are unclean.

Observed open insulation and missing ceiling tiles in the dry storage due to the fire incident. Ceiling tiles and insulation need fixed and repalced. Unapproved materials covering the walls and/or ceiling.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed dust and debris on the floor in the dry storage area from the ceiling tiles being removed. Floor, utensils and equipment in this area will need cleaned. Damaged or contaminated products will need to be discarded. Facility not maintained clean.

| We received notification that there has been a fire at the facility on 7/16. We went to the facility and talked to the manager, Nichole, about the situation. Facility was open at the time of inspection. The fire incident was outside of the building but ceiling tiles were removed inside the building at the dry storage area to ensure no fire or smoke was getting inside the building. Manager stated they started cleaning the dry storage area last night and was still cleaning this morning. Observed dust and debris in the dry storage area where ceiling tiles were removed. Observed ceiling tiles not replaced and open insulation in the dry storage. We talked to the district manager, Scott, and facility decided to voluntarily closed. Manager stated a cleaning company, PuroClean was at the facility today and will get back to them on the next steps. Manager stated area will be cleaned before and after the ceiling tiles have been fixed. Facility shall empty the Ice machine and have it cleaned before reusing it. Facility shall clean everything in the dry storage area and discard any damaged and/or contaminated food products, utensils (single use items) and equipment. Facility shall fix and replace ceiling tiles in the dry storage. After everything is completed, facility shall contact the Health Department for an inspection before reopening. |
