[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Corrected During Inspection TCS foods in the walk in cooler were not being held below 41F. PIC disgarded TCS items in the walk in cooler; lettuce, sliced cheese, raw meats, eggs. TCS foods were not being held at the proper temperature.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less.
3717-1-03.4(F)(2) / Time/temperature controlled for safety food - cold holding of eggs
Critical Corrected During Inspection Raw shell eggs were being held at a temperature of 60F. PIC disgarded the eggs. Raw shell eggs are not maintained at 45° F or less.
To prevent the growth of pathogens, eggs that have not been treated to destroy all viable Salmonellae shall be stored refrigerated at an ambient air temperature of 45° F or less.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Corrected During Inspection Observed sticker residue on the food contact pans. Spoke to the PIC in regards to making sure all sticker residue is removed before washing. Equipment food-contact surfaces or utensils are dirty.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-04.2(D) / Ventilation hood systems - adequacy.
Observed build up and debris on the hood vents. Explained to the PIC that the vents on t the hood shall be cleaned on a routine basis.Observed an accumulation of grease or condensation from ventilation hood system.
Ventilation hood systems and devices shall be sufficient in number and capacity to prevent grease or condensation from collecting on walls and ceilings.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the make tables hot holding proper temperature. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the walk in cooler not holding proper temps. PIC automatically put in a work order to ger the equipment working properly. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed the microwave with build up and debris. Observed accumulation of soil residue on nonfood-contact surfaces.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed build up and debris througout the facility. The whole facility needs a general cleaning. Observed accumulation of soil residue on nonfood-contact surfaces.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed multiple locations throughout the facility that needed cleaned. Make table lines for the toppings, ice cream needed cleaned. Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Repeat Observed trash around the dumpster. Observed an improperly cleaned storage area for refuse, recyclables, or returnables.
A storage area and enclosure for refuse, recyclables, or returnables shall be maintained clean and free of unnecessary items.
3717-1-05.4(B) / Outdoor storage surface.
Observed the refuse storage area with a cracked and damaged pad. Outdoor refuse storage surface is not properly constructed, and sloped to drain.
An outdoor storage surface for refuse, recyclables, or returnables shall be constructed of nonabsorbent material and shall be smooth, durable, and sloped to drain.
3717-1-05.4(N) / Covering receptacles.
Observed the dumpster lid open at the time of inspection. Observed waste receptacles that were not covered.
Receptacles and waste handling units for refuse, recyclables, or returnables shall be: kept covered inside the FSO or RFE if they contain food residue and are not in continuous use, and after they are filled; or with tight-fitting lids or doors if kept outside.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed build up and debris on the walls behind the dish wash line. Observed a build-up of dirt and debris.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
