[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical The employee sick policy was not verifiable at the tim eof inspection. PIC stated the binder containing the signed copies of the sick policy was discarded by mistake. A copy of the sick policy was provided to PIC at the time of inspection to make copies and have all employees sign it. Employees are not informed in a verifiable manner of their responsibility to report information about their health.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Corrected During Inspection Observed employee rinsing a container at the hand sink at the front. Talked to PIC about proper use of handsink. Handwashing sink is being used for purposes other than handwashing.

3717-1-04.5(B)(3) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Corrected During Inspection When asked PIC how often the knife used to cut TCS food items and stored at room temperature is cleaned, PIC stated every 8 to 10 hours. Talked to PIC about cleaning the knife every 4 hours. Equipment and utensils contacting TCS food not cleaned every four hours.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Repeat Corrected During Inspection Observed cole slaw cold holding at 49.5F and cut onions cold holding at 50F in the make table by the fryers. Cole slaw and onions were there for more than 4 hours. PIC voluntalry discarded them.TCS foods not being cold held at the proper temperature.

3717-1-04.1(DD) / Warewashing machines - automatic dispensing of detergents and sanitizers.
Critical Observed the dishwasher not properly dispensing the sanitizer at the time of inspection. Talked to PIC about having it checked. PIC put in a work order for the dishwasher at the time of inspection. Facility will be using the 3 compartment sink in the meantime. Warewashing machine does not automatically dispense detergent sanitizer and/or does not have a visual means to ensure detergent and sanitizer is automatically dispensed.

3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Repeat Corrected During Inspection Did not observe a thermometer inside the cold holding unit next to the grill, the make table by the fryers and the walk in cooler. PIC put one inside all of them. Food thermometer not readily accessible.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed uncovered food items in the 2 doors freezer next to the fryers and uncovered ice cream bucket in the ice cream freezer. Improper storage of food items.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Observed build up on the trays where single used conatiners are sored. Talked to PIC about cleaning the trays to prevent contamination. [52] Improper storage of single-service and single-use articles.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed the cold holding well and steam well next to the grill not working. Food is set up on ice in the cold well. The steam well was not being used at the time of inspection. PIC stated a work order has already been submitted to get it fixed. Equipment components are not intact, tight or properly adjusted.

3717-1-04.1(KK) / Food equipment - certification and classification.
Observed the use of non commercial Toshiba microwave. Equipment not approved by a recognized testing agency.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
Facility did not have QAC test kit for their sanitizer buckets at the time of inspection. No sanitizer test kit available.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed build up and debris in both microwaves in the kitchen. Observed debris in the gaskets of coolers and freezers. Observed build up inside coolers and at the bottom of the ice cream freezer. Non-food contact surfaces of equipment are unclean.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed dust accumulation on the vents cover of the walk in cooler. Ventilation system not maintained.

3701-21-03 / Facility layout and equipment specifications for FSO
Observed facilty has new smoothie machine without submiting specfications for prior approval. PIC provided specifications for added equipment at the time of inspection. FSO did not submit facility layout and equipment specifications.
