Critical Corrected During Inspection Observed two sanitizer buckets without proper concentration of QUAT solution. PIC had employees change water out and retest. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness.

Critical Corrected During Inspection Observed no date on the creamer machine in the service line. PIC placed a date on the creamer. Refrigerated, ready-to-eat, TCS foods not properly date marked.

Critical Corrected During Inspection Observed lettuce, cucumber and cheese without a date in the walk in. PIC threw items away. Refrigerated, ready-to-eat, TCS foods not properly date marked.

Observed no thermometer on the milk cooler in the self service line. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

Repeat Observed multiple items without a net weight. PIC stated she would add that to their labels from here on out. Label on a food packaged in-house did not contain all required information.

Observed a torn gasket of the grill freezer. Equipment components are not intact, tight or properly adjusted.

Repeat Observed vent grates in the walk in freezer to have some dust build up. Non-food contact surfaces of equipment are unclean.
