3717-1-02.4(A)(2) / PIC - Manager Certification
Did not observe a level 2 manager certification ODH certifcate on site at the time of inspection. PIC is currently working on getting ODH cert and has emailed them. Facility does not have an employee with manager certification in food protection.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Corrected During Inspection Did not observe a common name label on the rice, flour, and grits containers. PIC placed label on each container during the inspection. Working food containers not properly labeled.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Observed a cracked lid on the oatmeal container. Nonfood-contact surfaces constructed of unapproved materials.

3717-1-06.1(A) / Floors, walls, and ceilings.
Observed chipped and peeling floor in the dish room. PIC stated they are working on getting it repaired. Floors, walls, and/or ceilings not smooth and easily cleanable.
