Observed ice build up and condensation due to leaking in Freezer 1. PIC stated a work order is already in to have it fixed. Equipment components are not intact, tight or properly adjusted.

Observed broken and water damaged ceiling tiles in Dry storage 3. Physical facilities not maintained in good repair.
