Repeat Facility does not have a person with level two certification in food protection. PIC has level two Servsafe but does not have the ODH certificate at the time of inspection.

Repeat Equipment and/or components not maintained. Observed damaged gasket on the door of the freezer in the kitchen.

Equipment and/or components not maintained. Observed the make table in the kitchen with accumulation of water at the bottom. PIC stated the facility is getting a new make table. A spec sheet will be submitted to the health department.

Equipment and/or utensils improper construction. Observed craked lid on the cheese sauce container.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed food debris in the gasket of the freezer in the kitchen.
