[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Corrected During Inspection Observed mop sink and drain by coffee station with an insuffcient air gap at the time of inspection. PIC has notified maintenance. Will reinspect within a week. Insufficient air gap between the flood rim and the water supply inlet.
PIC effectively remedied the situation by making sure there was a sufficient air gap in both the mop sink and drain by the coffee station.

3717-1-04.1(Y) / Temperature measuring devices.
Repeat Observed upright cooler with no internal thermometer at the time of inspection. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed walk in freezer and wall freezer with a build up of ice at the time of inspection. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed build up of debris on multiple pieces of equipment. Non-food contact surfaces of equipment are unclean.

Repeat Observed multiple coolers with gaskets with a build up of debris. Non-food contact surfaces of equipment are unclean.

3717-1-05.4(O) / Using drain plugs.
Repeat Observed dumpster without drain plug. No drain plugs in waste receptacle.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed walk in cooler, back storage room and kitchen floor with debris at the time of inspection. Facility not maintained clean.

3717-1-06.1(G) / Walls and ceilings - attachments.
Repeat Observed vents in the stalls of both bathrooms with an accumulation of debris at the time of inspection. Attachments to the wall and/or ceiling were not easily cleanable.
