Critical Repeat Corrected During Inspection Observed a can of Skyline Chili with a large dent. Person-in-charge removed it from the shelf. Unsafe food was not discarded.
To prevent the transmission of foodborne illness, a food that is unsafe, adulterated, or not honestly presented as specified under 3717-1-03 of this code shall be discarded or reconditioned according to an approved procedure.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed build-up on the produce water sprayers. Equipment food-contact surfaces or utensils are dirty.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-03.5(B) / Honestly presented.
Repeat Observed ham sale tags without the full name of the product. Example Sara Lee Virgina Brand ham with water added.
Food shall be offered for human consumption that does not mislead or misinform the consumer. Food or color additives, colored overwraps, or lights may not be used to misrepresent the food. The sale of kosher foods shall be in accordance with section 1329.29 of the Revised Code.
3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Observed several meat trays cracked in the meat department. These trays are no longer able to be washed, rinised, and sanitized properly.
To prevent contamination, materials that are used in the contruction of utensils and food contact surfaces may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: Safe; durable, corrosion-resistant, and nonabsorbent; Sufficient in weight and thickness to withstand repeated warewashing; finished to have a smooth, easily cleanable surface; and resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

Observed build-up on the drain tray of the pop machine. Non-food contact surfaces of equipment are dirty.

Repeat Observed food spillage in the self-service meat coolers. Non-food contact surfaces of equipment are dirty.

Repeat Observed the air vents in the dairy cooler with visible build-up. Non-food contact surfaces of equipment are dirty.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Repeat Observed the vents in the back room of the bakery with visible dust, causing dust on the ceiling. The ventilation system was not being properly cleaned.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.
