3717-1-04.6(B) / Sanitizing frequency of utensils and food-contact surfaces
Critical Repeat The sanitizing solution is not being dispensed from the dishwasher. Utensils and food-contact surfaces of equipment were not sanitized at the required frequency.
To prevent pathogen growth, utensils and food-contact surfaces of equipment shall be sanitized before use and after cleaning.

3717-1-03.5(C)(3) / Food labels - bulk food
Repeat Did not observed an ingredient statement for the blueberry and chocolate muffins. Bulk food display for customer self-service was not properly labeled.
Bulk food that is available for consumer self-dispensing shall be prominently labeled with the information specified under (C)(2) of this rule.

Corrected During Inspection Observed the use of non commercial plastic food containers. Discussed with PIC about switching to commercial or placing food inside containers with the original package, Equipment is not approved by a recognized testing agency.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.

Observed the use of non-commercial crock-pot. Equipment is not approved by a recognized testing agency.

Repeat Observed no test kit to test sanitizing concentration. There is no test kit available for measuring the concentration of the sanitizer.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.
3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Corrected During Inspection Did not observed a thermometer inside the 2 doors cooler in the kitchen. PIC put one inside. A food thermometer was not readily accessible.
To prevent foodborne illness, food temperature measuring devices shall be provided and readily accessible for use.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat The dishwasher is not properly dispensing sanitizer. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Repeat Observed stir sticks that were not protected from contamination. Cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored.
