3717-1-05.1(D) / Backflow prevention - air gap.
Critical Observed no air gap between flood rim and water supply inlet during the time of inspection. Insufficient air gap between the flood rim and the water supply inlet.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Observed soda concentrate to not be stored 6 inches off the ground. Improper storage of food items.PIC corrected this by placing it on shelf.

3717-1-03.2(K) / In-use utensils - between-use storage.
Corrected During Inspection Observed fryer utensil stored with handle down during the time of inspection. In-use utensils improperly stored. PIC corrected this by placing utensil properly with the handle in the air.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Observed hand sink in the front of kitchen to be leaking when turned on during the time of inspection. Plumbing system not properly maintained or repaired.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. observed damaged ceiling tile in the dining area during the time of inspection.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed vent covers in both the men's bathroom and above the roast beef cutter to have debris buildup. ventilation system not maintained.
