3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Did not observe lids on the bacon and the sugar containers during the time of inspection. Food not properly protected from contamination by separation, packaging, and segregation. PIC placed lids on bacon containers and threw away sugar and stated they are not using it at the moment.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Corrected During Inspection Did not observe proper date marks on several items in the walk-in cooler during the time of inspection. Refrigerated, ready-to-eat, TCS foods not properly date marked. PIC disposed of items.

3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Corrected During Inspection Observed bacon cutter to be sitting on dirty paper towels during the time of inspection. Food contact surfaces not easily cleanable. Explained to PIC that the paper towels are absorbing bacteria and not capable of being cleaned to the required standards provided by the health code. PIC removed paper towels, cleaned bacon slicer, and placed slicer on the proper surface.

VR 04 / Monitoring procedures being followed
Observed there to be no time stamp on the cheese slices during the time of inspection. Per the variance approval letter, each container of cheese slices being held at room temperature shall be marked or otherwise identified to indicate the time that is eight hours past the point in time when the food is removed from temperature control.

Corrected During Inspection Did not observe thermometer in the walk-in cooler during the time of inspection. Food thermometer not readily accessible. PIC placed thermometer in cooler.

3717-1-06.1(M) / Outer openings - protected.
Observed the back door sweep to be damaged and light to be seen under the door during the time of inspection. Outer opening not protected.

3717-1-03.2(K) / In-use utensils - between-use storage.
Corrected During Inspection Observed bacon slicer to be stored on paper towels during the time of inspection. In-use utensils improperly stored. PIC cleaned slicer and stored it on a proper surface.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Corrected During Inspection Observed old date stickers that were still on the non-food portions of the food containers during the time of inspection. Non-food contact surfaces of equipment are unclean. PIC removed containers to have them cleaned.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed coving on hand sink to be damaged during the time of inspection. Floor and wall junctures not properly coved or closed and/or floor drains not provided.
