Repeat Facility does not have a person with level two certification in food protection. PIC did not have the ODH certificate. PIC is scheduled to take the December servsafe class at the health department.

Food thermometer not readily accessible. Observed misssing thermometer inside the cooler in the kitchen.

Equipment and/or components not maintained. Observed torn gaskets on both coolers at the bar.

Non-food contact surfaces of equipment are unclean. Observed build up at the bottom of the cooler and on the door gaskets.
