3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Critical The plumbing system was not properly repaired. There is a pipe coming out of floor by cola machine in back that has duct tape covering the opening that has ripped. Water inside smells putrid. Observed maggot casings on tape and something appeared to be growing at the surface of the water.
A plumbing system shall be repaired according to the Ohio building code. Have a registered plumber inspect and make repairs as needed. Correct By: 25-Aug-2014

Critical No toilet tissue in one stall of womens restroom.

3717-1-06.4(K) / Controlling pests.
Critical Observed the presence of live insects, rodents, and other pests. Observed flies and gnats around friers, milkshake machine, back room by cola machine, dining room tables, soda dispenser in lobby, and throughout facility. Facility installed enclosed bug zappers last week-evidence more needs done-possibly more often cleaning, check drains, etc.
The presence of insects, rodents, and other pests shall be controlled by routinely inspecting incoming shipments of food and supplies, and routinely inspecting the premises for evidence of pests, using methods such as trapping devices or pesticides, and eliminating harborage conditions.
